Equipment Loan to Students

Does anyone have a document they have parents sign if a student is loaned/assigned a piece of school property?  We are looking a loaning a laptop to a student that is having surgery and I'd like the parent to assume responsibility if the machine is lost/damaged etc.

We had the students sign an agreement when they picked up their netbooks for the 10-12 one to one program that began this school year. For special cases (students or staff signing out equipment) I use variations of the file I uploaded to this post: "The Dover-Eyota Equipment Use Checkout Agreement"
